New Patient Forms

New Patient Form

We are currently accepting new patients. We ask that you bring the completed new patient form with you to your first visit. 

When coming in for an initial evaluation we ask that you bring:


  • A photo ID
  • Your insurance card
  • Questions for the doctor
  • A new patient packet filled out
  • Prior to your visit we ask that you have your primary care physician or previous pain physician fax us the last 5 office visits as well as any imaging studies performed for your symptoms.
On your initial visit to Omega Interventional Pain, you will be evaluated by a Medical Doctor (M.D.) who has received specialized training in the treatment of acute and chronic pain. A thorough treatment plan will be created with some of the initial steps in your rehabilitation placed into action.

Please note we do not prescribe opioid medications on your initial visit.
