Patient Transition Notice

August 31, 2026

 

Dear Omega Patient,

 

I want to personally share an important update about Omega Interventional Pain Management and your ongoing care.

 

After many years of caring for our patients, Omega Interventional Pain Management is transitioning its clinical operations and patient care to Interventional Spine and Pain Management (ISPM). We recognize that changes in your healthcare can create questions and uncertainty, and our priority is to make this transition as smooth and seamless as possible.

 

Many of you have trusted me and our team with your care for years. We are committed to helping ensure that your medications, procedures, follow-up appointments, medical records, and ongoing pain-management needs continue with as little disruption as possible.

 

What This Means for You

 

Our goal is to have all Omega patients transitioned by September 30, 2026. You may choose to:

  • Continue your pain-management care with ISPM, where many members of the Omega team will continue to be involved in your care; or

  • Transition to another physician or pain-management practice of your choice.

For most patients who choose ISPM, your care can continue with minimal disruption. I also plan to begin my own clinical work with ISPM after October 15, 2026.

 

Through September 30, 2026:
Please continue to contact Omega Interventional Pain Management at 801-261-4988 for your medical records, transition assistance, appointments, and services provided through Omega.

 

Beginning October 1, 2026:
Please contact Interventional Spine and Pain Management (ISPM) at 801-262-7246 for access to your medical records and for ongoing pain-management services.

Your Choice of Provider

 

You are always free to choose the physician or pain-management practice that you feel is best for you. There is no obligation to continue your care with ISPM.

 

If you choose another provider, our team will be happy to assist with continuity of care and the transfer of your medical records after receiving the appropriate authorization. Our goal is to support you in making the decision that is right for your healthcare needs.

 

Medical Records

 

Through September 30, 2026, Omega will remain your point of contact for medical-record requests and transition assistance.

 

Beginning October 1, 2026, ISPM will assume responsibility for the ongoing retention and access of Omega patient medical records. At that time, requests for copies or transfer of medical records should be directed to ISPM at 801-262-7246.

 

We will work together during this transition to ensure your records remain available and can be transferred to the provider or practice you choose.

 

Billing and Account Questions

 

Charges and account balances for services provided by Omega on or before September 30, 2026 will remain with Omega.

 

Beginning October 1, 2026, services provided through ISPM will be managed and billed by ISPM.

 

If you have questions about billing, payments, claims, or account balances during the transition, please contact the office responsible for your care based on the dates above. Our teams will work together to help direct your questions to the appropriate office.

 

Important Information for Intrathecal Pump Patients

 

If you have an intrathecal medication pump, please pay particular attention to this section.

 

Your pump requires ongoing, specialized management, and we want to make sure there is no interruption in your pump care during this transition. Pump management will need to transition to another qualified provider who offers intrathecal pump services.

 

Until that transition has been successfully completed, Omega will continue to provide necessary pump refills, programming, and related pump care.

 

Please do not wait until your pump is approaching its low-reservoir alarm or until you have an urgent need to begin this process. We strongly encourage you to start arranging your transition to a qualified pump provider as soon as possible.

 

If you are an intrathecal pump patient, please contact Omega at 801-261-4988 on or before September 30, 2026 so that we can help coordinate your transition and make sure your pump-care needs are appropriately addressed.

We understand that continuity of care is especially important for patients living with chronic pain. Our goal throughout this transition is to keep your care moving forward safely, communicate clearly, and help you avoid unnecessary interruptions in treatment.
